Xtreme Phaze is a dba (assumed business name) of Eternity's Image Ministries. Eternity's Image Ministries is a integrated auxiliary of the Church and is not operating a commercial store or mercantile establishment at this or any other location.

This is a Church recreational facility where fees are charged and items may be sold in exchange for recommended donations. Anyone unable to make a donation may submit an application for the use of our facilities or for items to be made available at a reduced or no expected donation.
